New patients, save time in the office by downloading office forms here. Fill the forms out using black ink and bring them with you to your appointment. (We scan your forms into your medical record and the ink must be black to scan.) Please bring your insurance information as well as a current medication list, too! If you are a contact lens wearer include your lens solutions.
