Welcome to Eye Tech Eye Associates!   We are a family oriented full service optometric practice.   At Eye Tech, we set a high standard for your eye care needs. Our goal is to provide an eye exam that goes far beyond “routine” using the most technologically advanced tools available.  We have been serving Peoria, Glendale, Sun City and surrounding areas since 1993.

New and Returning Patients:  save time in the office by downloading registration forms here.  Fill out the forms using black ink and bring them with you to your appointment.  (We scan your forms into your medical record and the ink must be black to scan.)   Also, please bring your insurance card and a list of your current medications.    If you are a contact lens wearer, please bring in the packaging from your current lenses as well as the solution you use.
